About Good Faith Estimates

Dear Patient,

You have the right to receive a Good Faith Estimate explaining how much your medical care will cost.

• You are entitled to a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs such as medical tests, examinations, office visits, prescription drugs, and equipment (items or services reasonably expected to be furnished by this practice).
• Your health care provider must give you a Good Faith Estimate in writing at least one (1) business day before your scheduled service or item. You may also request a Good Faith Estimate before scheduling.
• If your bill is $400 or more than your Good Faith Estimate, you have the right to dispute the bill.
• Please be sure to save a copy or photo of your Good Faith Estimate for your records.
